An Accidental Repetitive 10-Fold Overdose of Sildenafil in a Young Infant with Pulmonary Hypertension

Sildenafil is a selective phosphodiesterase type-5 inhibitor that increasingly used to treat pulmonary hypertension (PH) in neonates. Only little known about the relation between dose of sildenafil, plasma concentrations, and degree toxicity. Here, we present young infant with congenital diaphragmatic hernia PH who received an unintentional 10-fold overdose oral sildenafil for 6 consecutive days. This overdose, compared therapeutic dose, resulted increased concentrations from 42 521 mcg/L desmethylsildenafil 81 393 mcg/L. However, high exposure only led diarrhea, without any other serious adverse events. case describes mild symptoms upon role drug monitoring monitor therewith support clinical decision-making.
